see the sample ouput carefully
your output is reverse of what is asked

you have to just reverse the order of recursion calls you get correct ouput
like this:

generate_subsequences(t,out,i+1,j);
     out[j]=t[i];
    generate_subsequences(t,out,i+1,j+1);

you can see Modified Code this is passing all testcases
